Intended Parents Information
Dear Intended Parents:

Thank you for your interest in my program. I know there are other Surrogate and Egg Donor programs, and I welcome the chance to describe mine.

Most of my Egg Donors and Surrogates are in Southern California. Some are married, some work outside the home, and they come from a variety of ethnic backgrounds.

Most of my Egg Donors have children, or have been Egg Donors before. They are between the ages of 18 and 34, and have a healthy height/weight range. The most common reason for becoming an Egg Donor is “to help.”

All of my Surrogates have given birth, and they fall between the ages of 21 and 43. Their reasons for becoming a Surrogate vary--the most common reason is to help someone make a family (many Surrogates know someone experiencing infertility, and feel they can make a difference by becoming a Surrogate).

(Please remember other Intended Parents are reviewing profiles, and may select the same Egg Donor and/or Surrogate you have selected.) Select two or three prospective Egg Donors and/or Surrogates whose profiles you would like to see. When you have reviewed the profiles of the Egg Donor and/or Surrogate you are interested in meeting, contact me, and I will call the women and tell them briefly about you (if none of the profiles interest you, I will be happy to send others).

Your Egg Donor and/or Surrogate are screened medically and psychologically before they begin any medications.

(Egg Donors usually do not meet the Parents, although you may ask to meet the Donor if you wish), in a neutral place--and I will attend. I will make arrangements for you to meet 1 to 3 Surrogates. After the meetings, you are given a few days to decide if you want to work with any of the Surrogates. If you and she agree to work together, you will need to go and have the necessary medical tests administered. The Surrogate and her husband, if any, will also have the same tests run on them. The Surrogate and her husband will also have a psychological evaluation.

When all test results come back in your favor (and in the Egg Donor’s and/or Surrogate's favor), you will be told what medication they need to use to align your cycles.

When cycles are aligned, the doctor will perform the procedure. Approximately two weeks later, blood tests show if you or the Surrogate is pregnant.

After confirmation of pregnancy by the transfer physician, the Surrogate may see the Obstetrician of her choice. These appointments will be paid for by her insurance. Any amount her insurance does not pay is paid through the Trust Account established by you.

The Surrogate's pay comes from a Trust Account. She will be paid monthly, for a maximum of twelve months. She is given an allowance each month for maternity clothes, travel, and childcare. She does not send receipts for reimbursement (except for doctor's bills and prescriptions). Any amount she spends in excess of the allowance is her own expense. If she does not use the entire allowance, she may keep the excess.

After confirmation of pregnancy, the Surrogate will attend monthly therapy sessions. These meetings will help her to know what to expect and how to deal with certain situations. The meetings will continue for three months after the birth of your child. The Surrogate is compensated for these meetings. If she does not attend the meeting, she receives no compensation.

Enclosed is a list of approximate costs. Costs have been rounded-up, and some fees that cannot be estimated (i.e. medications) are not listed. The total estimated cost of the Egg Donor and the Surrogate fee, must be in the Trust Account before your Egg Donor and/or Surrogate goes through the IVF procedure. If you need time to arrange this, I will hold your agreement for a maximum of one year from the date of signing.

When we sign the agreement, Cori’s Egg Donor and Surrogate Services, Inc. fee is due. This amount ($10,00.00) for a Surrogate and ($ 3,000) For Egg Donor is included in the total cost, and is the only fee not taken from the Trust Account. The Egg Donor and/or Surrogate's costs and expenses, are paid through the Trust Account. If you have any questions about my program, please feel free to call.
